Position:  Permanent Full Time

Department: Programs- Reporting to the Program Manager

JOB SUMMARY:

Elm Grove Centre is 126 bed long term care home that is dedicated to providing  a holistic, resident focused approach to care. We are located in Toronto close to the King and Dufferin area. We currently have an opening for a permanent full time activation assistant.

Responsibilities:

-          Participate in planning, implementing and evaluating a wide range of programs in the home including 1:1, small group, large group, focus groups, and special events that meet the physical, social, intellectual, emotional and spiritual needs of our residents.

-          Complete resident assessments and establish goals and interventions based on the residents identified individual needs.

-          Complete program documentation including initial assessments, attendance, restorative care, progress notes, care plans and assessments and raps using RAI/MDS.

-          Assist in the development of the monthly calendar.

-          Decorate for special events, holidays and seasonal events.

-          Comply with all company policies, procedures and practices.

-          Other duties as assigned

Qualifications:

-          Diploma or degree in activation, gerontology or therapeutic recreation from a recognized college/university.

-          Experience working in a long term care setting.

-          Must have effective verbal and written english communication skills.

-          Must possess effective leadership, organizational and interpersonal skills.

-          Must demonstrate initiative, and the ability to work in a team setting or independently.

-          Must have experience in program planning, documentation, and evaluation.

-          Experience with RAI/MDS is an asset.

-          Experience working with people with mental health issues is an asset.
